Our care includes a team of doctors, nurses, social workers, counselors, hospice aides, chaplains, and trained volunteers. They work together focusing on the patient’s needs, whether physical, psychological, or spiritual.
To help provide care and support that reflect the important contributions made by these men and women, all of our Grace Hospice locations have become national partners of We Honor Veterans, a pioneering campaign developed by National Hospice and Palliative Care Organization in collaboration with the Department of Veterans Affairs.
At Grace Hospice we understand that grief is a journey. The goal of our Bereavement Program is to help guide you along this journey and support you as you learn how to adjust to life without your loved one.
Our organization offers the unique advantage of providing patients with a continued close relationship with their primary care physician, and also the ability to connect patients and their families with a physician who offers house calls.
